A Support Coordinator helps people and families get the very most from their NDIS funding.

Case Study

Name: Shireen

Age: 24

Background: Shireen was diagnosed with Cerebral Palsy just before her first birthday. She received regular Physiotherapy, Occupational Therapy and Speech Therapy support as a child, and now as a young adult wants to build independence so that she can live away from her family home.

Support Coordination: Shireen’s Support Coordinator Jacky identified that she would need to see an experienced Occupational Therapist for a Functional Assessment, as well as a Home & Living Suitability Assessment so that Shireen could request for funding under the NDIS Supported Independent Living (SIL).

Shirleen’s Support Coordinator was able to organise for an Occupational Therapist specialised in Accommodation through her network to visit Shirleen at her place. They went through an extensive assessment to help identify Shirleen’s care requirements when she lives away from her family.

Jacky also reached out to different SIL providers to find suitable accommodation at an area not far from home and was able to find accommodation right away. From the moment she set foot in the house, Shirleen was in love — it was the perfect match for her.

Moving quickly, and with assistance from the OT and SIL provider, Jacky organised the documentation required to be submitted to the NDIA. Very soon, they received a call from the NDIA planner, and Jacky supported Shirleen throughout the plan review process.

Fast forward two months, Shirleen now lives comfortably in her new home with 24/7 staff and enjoys regular visits from her family. Shirleen, with the help of Jacky, has learned a lot through the process and felt that she has been given the empowerment to be as independent as ever.
